Clinical Running Essentials – Migrating Runners
Out of the Medical System

Date: November 9-10, 2024
Instructor: Chris Johnson, PT
Location: Bozeman, MT

A two-day, hands-on course for clinicians to improve their ability to rehabilitate runners. It’s aimed to help clinicians better manage running-related injuries and guide athletes back to peak performance.

Course Description

Running is a practical exercise mode, considering its wealth of documented healthbenefits and minimal barriers to entry.

Running, however, is associated with a highincidence of lower extremity injury, with studies highlighting that nearly eighty percent of runners are likely to sustain an injury over a year.

Most running-related injuries (RRIs) tend to be multi-factorial and characterized by overuse and take the form of tendon-related disorders, bone stress injuries (BSIs), and fasciopathies.

Symptoms associated with RRIs are often recalcitrant and tend to recur if treated inappropriately. Some athletes may be fortunate to return to training promptly, whereas others are often sidelined for weeks, months, and sometimes years.

In extreme cases, certain athletes may have to retire or discontinue sport if managed haphazardly; if not go on to develop life-altering impairments and sequelae that have the potential to shorten their lifespan and compromise their reproductive health.

Clinicians looking to develop proficiency in helping runners with these conditions must have a command or knowledge base on several domains.

While there is still a great deal to learn, this course provides clinicians with an evidence-based framework geared towards helping clinicians better manage runners and ultimately migrate them out of the medical system and back to consistent and healthy training.

Course Structure

An eclectic, live, 2-day course that draws on the best and most current research related to the following domains:

  • Biomechanics and performance demands of running.
  • Motivational interviewing.
  • Therapeutic alliance.
  • Orthopedic evaluation and physical performance tests specific to running.
  • Running gait analysis and gait retraining
  • Drills for running mastery
  • Autoregulatory principles related to running and resistance training
  • Rehab pillars in managing bone stress injuries and lower limb tendinopathy.

This course is geared towards clinicians with an intermediate level of experience, which assumes that the participants are generally familiar with the topic, so it focuses on refined understanding and application. This course offers an intimate virtual learning environment while affording ample time for questions, interaction, and reflective
learning.

Course Objectives

Summarize the fundamental tenets of motivational interviewing and therapeutic alliance to promote patient autonomy. Learning Strategy/Instruction Method: Lecture with question and answer, demonstration, and role-play.
List critical run-centric questions to ask during the subjective assessment to ascertain a runner’s general health and well-being and to determine their relative risk for bone stress injuries and potential adverse life events. Learning Strategy/Instruction Method: Lecture with question and answer.
Describe the biomechanics, EMG, kinematics, and kinetics of walking and running while highlighting the key differences between these two modes of locomotion. Learning Strategy/Instruction Method: Lecture with question and answer.
Explain the incidence and risk factors of running-related injuries and the regions most likely affected. Learning Strategy/Instruction Method: Lecture with question and answer.
Identify the severity and drivers of running-related injuries while screening for red flags and staging and typing RRIs per the Modified Wilk Scale. Learning Strategy/Instruction Method: Lecture with question and answer and case studies.
Introduce “The Table Test” and assess its impact on clinical decision-making, manual interventions, and exercise prescription. Learning Strategy/Instruction Method: Lecture with question and answer, demonstration, and practical exercises for participants.
Identify key physical performance tests (PPTs) to establish a baseline capacity while screening and assessing for soft tissue injuries and bone stress injuries (BSIs). Develop an exercise program based on “The Ls of Loading” based on PPT results. Learning Strategy/Instruction Method: Lecture with question and answer and live demonstrations.
Discuss high and low-risk BSIs, their physiological differences, and their implications on the rehabilitation process. Learning Strategy/Instruction Method: Lecture with question and answer.
Describe, demonstrate, and provide cueing for running mastery drills and functional exercise progressions incorporating neuromotor training, compound movements, and power/plyometrics. Learning Strategy/Instruction Method: Lecture with question and answer and real-time demonstrations.
Differentiate the three common strike patterns related to running and analyze their respective implications for injury and performance. Learning Strategy/Instruction Method: Lecture with question and answer.
Conduct a comprehensive running gait analysis using “The S’s of Treadmill Running Analysis” and offer specific interventions from a gait retraining standpoint for various contexts. Learning Strategy/Instruction Method: Lecture and demonstration by the instructor with question and answer, followed by lab time for participants to conduct a treadmill analysis.
Design and implement a return to running program using a graded exposure model to safely return runners to training and competition, specifically for lower limb tendinopathy and BSIs. Learning Strategy/Instruction Method: Lecture with question and answer and case study.
Define high versus low-risk bone stress injuries, their physiologic differences, and projected timelines for recovery. Learning Strategy/Instruction Method: Lecture with question and answer.
Discuss when imaging is appropriate for lower limb tendinopathy and bone stress injuries and select suitable imaging modalities based on the involved tissue(s). Learning Strategy/Instruction Method: Lecture with question and answer and sample cases.
Develop strategies for nutrition and address Relative Energy Deficiency in Sport (RED-S), considering their role in BSI development. Learning Strategy/Instruction Method: Lecture with question and answer.
Design loading strategies across the lifespan to optimize musculoskeletal health and safeguard against bone stress injuries and lower limb tendinopathy, integrating autoregulatory principles and anchoring techniques. Learning Strategy/Instruction Method: Lecture with question and answer and real-time demonstrations.
